Law, Bellaire and Tirkos South East Toronto Family Health Team The South East Toronto Family Health Team (SETFHT) is an academic Family Health Team affiliated with the Department of Family and Community Medicine, University of Toronto. We are a team of health care professionals providing a full range of primary health care services and programs to the residents of South East Toronto. The South East Toronto FHT includes a team of doctors, medical residents, nurse practitioners, nurses, dietitians, social workers, pharmacist, care navigator, mental health/addictions counsellor, physician assistant, chiropodist, health care professional students/interns and a full administrative team.
